When you fill Step-I online and pay the exam fees you will get a page at the last print it out that is form 186 you have to fill the necessary blank areas and get signed it from your principal or dean of your school or any government officer. Then download form 344 and 345 and tick mark at appropriate areas. Then enclosed all this three form along with the following documents
- Form 186
- Form 187
- Form 344
- Form 345
- Two copies of Medical Diploma certificate (awarded by the university – convocation certificate)
- Two copies of Transcript certificate
- Two copies of recent photograph
- Other you can also send internship completion certificate and clerkship document issued by your school not asked by the ECFMG but you can send them they will discard if they don't need it

I think you have to correct it, go to your university/medical school and submit an application for correction and get the valid documents from there that would be more wise.
