Docs Halt Services in Seti Zonal Hospital

All services at the Seti Zonal Hospital (SZH) were halted on Friday after relatives of a deceased beat up SZH medical officer Dr JB Thapa, accusing him of not providing proper treatment on time.

One Dipendra Ghimire, who was admitted to the hospital after being hit by a bus, died while being taken to India for treatment on Wednesday.

A bus hit Ghimire, who was returning home on his motorbike from Purano Bhansarchowk of Dhangadhi, at 9 pm on Wednesday. He was taken to SZH where Dr Thapa treated him and told Ghimire's relatives to take him to a hospital having the facility of a CT scan.

Injured Ghimire was taken to Kohalpur Medical College at 10 pm the same day but later died while he was being taken to Lucknow in India.

Some of his relatives this morning called on Dr Thapa at the emergency ward and beat him up, the hospital's medical superintendent Dr Arun Kumar Chaudhari said.

An emergency meeting of doctors was called immediately after the incident and the doctors demanded action against the guilty, a hospital source said.
Meanwhile, the relatives of the deceased are keeping his body at the entrance of the hospital and demanding compensation and action against Dr Thapa. They have also padlocked the entrance of the hospital.

"As there was no facility of a CT scan here, I referred the patient to another hospital," Dr Thapa told this daily. Security personnel have been deployed in the hospital for security.

Prem BC, a relative of Ghimire said, Dipendra died as Dr Thapa did not treat him on time and that the agitation was staged to protest against his "irresponsible act." - The Himalayan Times

What the heck is this?
We have another example of vandalism against doctors.
Is it true that Dr did not give treatment on time? If its true then NMC must take against him. Probably the paient might had some epidural or subdural hemorrhage which has poor prognosis if urgent neuro consultation is not done.
If not then its high time docs, lets unite and show these Nepalese our power of unification as they have been showing to us. Its they who are going to suffer.

Doctor is not GOD who can treat all. Well I don't know what was the severity of the patient after the trauma. But it seems there might have head injury and if the patient is unconscious CT is of course needed to diagnose and rule out other possibilities. If SZH has no facility of CT then the referral is possible after quick Emergency Physical Examination and vital support. Also from above it seems the patient died after taken to 2nd referral hospital- "Injured Ghimire was taken to Kohalpur Medical College at 10 pm the same day but later died while he was being taken to Lucknow in India." So it seems that the delay is not in SZH to refer.
Being a doctor doesn't mean that every disease can be cured or every fatal outcome can be totally prevented. Doctor can diagnose what might have happened or what is the cause, but the treatment depends on the medicine and instruments available at that time. Also the treatment depends on the nature of disease. In trauma like here the first thing to notice whether the patient conscious or not. If conscious then first referall to another hospital will be bad choice. In this case discussion with the senior should be taken if in doubt like help from neurologist (in this case). If there seemed other organ injury (Abdominal organs which are most common) or fractures then quick referral to the concerned department should be done admitting the patient in the hospital.
The quick CRASH PLAN examination will reveal much information about the injury, and GCS might help to predict the severity of brain. Airway,Breathing and Circulation should be secured and vital signs should be monitored. After priliminary diagnosis then quick referall to the concerned department is done. This is what a doctor can do in emergency department. For definite diagnosis instrumental examination is necessary. CBC, X-ray, CT, MRI, Endoscopy, USG etc. should be chosen according to its sensitivity and specificity. For the head injury CT or MRI is the best. X-ray is not sensitive or specific as CT/MRI.
In case I was present at that time in the emergency department, I would have done the same . I would have refered the patient to the concerned department after priliminary diagnosis and vital support. Here it seems logical to referred to advanced facility hospital since SZH has no advanced instruments like CT/MRI.
So I feel sad that patients relative felt it was a mistake of a Doc. They should think why there was need for 2nd referall to Lucknow? The time delay is the cause of death.
