Doc-less hospital turns away hundreds

ILAM, June 24, 2006 - Hundreds of patients visiting Ilam District Hospital have been deprived of treatment as there has been no doctor there for some one-and-half months.

The 25-bed hospital has three posts for doctors. Dr Tara Nath Pokharel left for Kathmandu on "study leave". However, his leave application is yet to be approved by the authorities, said a source. The other doctors have been absent for long.

"Doctors are not available here and we are unable to cure you. Better go somewhere else in time," Anjana Budhathoki of Golakharka quoted nurses at the hospital as telling her. Pregnant Budhathoki visited the hospital Friday for her check-up.

how come people don't go there for their internship? it's like places in ktm have limited places, why aren't people trying to district hosps?

I can't understand it either. I am personally sure that I will do half of my elective and Internship in Nepal and half in europe somewhere(if not germany). Students are motivated here to take some practicals in countries where hospitals are not sufficiently equipped to learn how you can perform your job with almost bare hands ( in lack of modern equipments). I have read quite a lot article of german students who did their elective or intern in Nepal admiring doctors there, who could perform operations so efficiently in a classical OP room. I think we should not always think that there is nothing to learn in remote. Moreover we have some responsibility towards our society. At least one month out of 1 year internship can be done in remote, I am sure. This is what a student can do. By the way, politically the situation has also become better.
However when the complete system is ill, students can hardly do a lot, but sofar they can. This news is not a new case. It is happening since a long time. I myself knew it as a school-boy. In my hometown's zonal hospital, there was posting for 18 doctors, but you could hardly see more than 4. Most of the patients could not help fleeing to india, since big indian cities are pretty near and you get better treatment. MoH never bothered itself to take action. At least I have never heard. And finally who are the victims - like always, poor people. Their lives does not mean a thing for the government. A traditional way of Ignorance.

yeah and we have all these medics saying that they want to do something for nepal, but when it comes down to the crux of the matter...where are they all? Kathmandu upateka isn't nepal...so what's happening all the newly graduates. there are currently 11 med schools in nepal each producing 100 docs, and also lots and lots of others in india, pakistan, bangladesh, china and russia...so it's not short of manpower really. so where is everyone going??!!

regarding doctors running away from -district hospitals-, it's the cause of government weakness in taking actions & also pondering upon the reactions made by the medical professionals in our country i suppose.. i hav heard about doctors rushing to their private clinics immediately after morning -rounds-, leavin everyt'ng behind in the naive&nervous hands of the interns in many hospitals.. this does nt sound fair at all to neither the interns nor to helpless patients of course.. many may think ok, this leaves interns with more chances of practicising on their own but in my view how safe is this on-our-own practice, in absence of any teachers around to make us do things, watch us do things & to correct us, guide us.. what i mean here is that as long as there are nt anyone to guide us, i don't think interns can learn as much as they should hav learnt with the guidance of senior doctors. so what use of interns going to the -district hospitals-; it only adds imperfection to their not perfect learning phase.. guidance from senior doctors is very crucial i feel.. even then many responsible people tend to be selfish & get away for their self fulfillment only like that Dr Pokharel in the above news piece..

This again comes down to who is at fault.But like everything else ,its the system that is at fault.Its funny to say why are interns not there in a district hospital because u are not supposed to take care of pts fully during internship,u are supposed to treat under guidance,i guess its obvious what sort of a guidance that we will get there.
Say I tomorrow say send me to ILAM i will go,do you think i can go?First there is the matter of number of "quotas" for the district.If its just one then its filled with that other doc.It takes more than 3 months for our gov.to recruit in the first place that is if they think it is necessary to do so.
Say I say,I want to go and open a nursing home there.There is one there already,it has been closed so many times that the doctor who opened it is in ktm more than in ILAM.If anything happens to any patient what ever the reason,the end result is....beating of the doc. and padlock to the clinic.How can a sane person work in such a condition.Constant irritation/trouble with maoists is too much well known to be discussed again i guess.
Its interesting that one of my uncles was a doc there 9 years ago and was doing fabulous job,he was transferred from that place to some other village because PM was visiting that place,how foolish is that.And he said,do hell with it all,i am over with all this!!
The moral of the story is frns its easy to say from far away.I was so patriotic during my college days that i was given the title "The Patriot" in my medical school.I used to plan which village i am going to work in,once i pass out.But once you pass out,work with our gov.functionaries u realise its a mess,earlier u comeout of it,more chances u have of retaining your sanity.Untill and unless health becomes a priority of our Gov.,untill and unless sane ppl rule our country its impossible to work.At least i couldn't.

So what is the solution ?? See the current gov won't move its ass, coz it is INTERIM, and they are busy in Talks with Mao. When will these addle eggs excuse our country ? Damn frustrating.

One more aspect of all this is that we cannot always rely on newspapers also.I am saying this because my best friend also works in the Gov.service and he was working in far west and was doing fine.He came to KTM for his marriage and stayed for some 15 days and it was written in Kantipur that he was out of station for more than 6 months .He found out that the NEPALGUNJ SAMBADATA of kantipur writes his story from Nepalgunj depending on what the paramedics of district hospitals tell him.My friend had problem with a senior paramedic coz he being the doc was the district health incharge and had to look over administration job too and found out so many irregularities of that guy.And that guy took a sweet revenge by calling the kantipur guy and telling him there is no doc in the district since six months and he believed and published it.
When my friend went to meet that Journalist and told him how can you write such stupid things,u know what he said.Its actually good for docs because now the gov.will think of increasing the Quotas because of his story.What sort of an answer is that.He should have Pleaded and said i am extremely sorry for this.Now that my friend had a bad stint with him.His name was constantly in Kantipur and once in "SAMPADAKIYA" too.He sometimes jokes and says.....ppl dont recognise him by face but once he spells out his name everyone knows him.HE IS FAMOUS......BY DEFAULT.

Why didn't he go to court ?

But there are mainly true cases regarding this topic. I have already given example of my hometown.
