A person who let his visitor put glass stick in the left armpit for about 5 minutes. In between these 5 minutes he first catches the left lower arm of the visitor and look at watch bewildering the visitor. Then after half minute catches right lower arm and looks at the visitors abdomen/chest making the female patient embarassed. He let the embarassed patient to remove the outer jacket so that only T-shirt left making upper arm naked. Then he wrap the right upper arm with some black cloth piece which is attached to somewhat like tiffin box but not having tiffin inside . Instead there is a black rubber ball which is very soft to press. He open the outer layer and put vertically and then start to play with ball. The visitor is excited to see what he is doing, while he continues to play the ball with right hand and feel the visitor's right upper arm below the clothpiece. He seems not satisfied with that so take out earphone like thing and place it at the place where he had put his left index and middle finger just before. After that he tells the patient to give his glass tube back.
Now he starts interogating the visitor about the visit. He doesn't seem to be harsh, but gentle and let the visitor to say his things in his own way. After listening about 5~10 min he then ask wh- questions. Slowly move to the private things. First start from chief complaint. Then visitor's present status, past status, systemic functions, personal matters. He is so keen to know everything that he even asks about girls private flowing with extra detail no matter how embarassed the girl feels about it. It seems he is very happy to make the girl embarassed. He is not only satisfied with the visitor's matter but also his family and relatives. Anyway after about 20 minutes he gets satisfied.
But it won't last too long. He wanna see what the reactions of the visitor if he touch him/her. Well let you guys make clear at first, he is neither gay nor les nor bi. He is a doctor and it is not a big deal that if by touch of him the visitor's problem is solved. Obviously the visitor show some reaction to his touch.
Hie is not satisfied yet. He then collects your blood, urine, stool even your meat. Also let you go through rays which may be X, CT, radioactive, magnetic etc.
After this he becomes bit tired with you and give some conclusion that the visitor has something abnormal. What abnormal? He then searches in his mind, books etc, consult with teachers, collegues, seniors etc, and finally make a decision. This decision he terms as final diagnosis.
He then writes worse than a nursery baby which is amazingly understood by his collegue working at drug station. He then give this drug orally or by syringing the visitor. He doesn't care whether it pains or not. If pain then he directs lady in a white gown to inject another syringe saying that syringe will let off your pain. The visitor puzzeled at first and scold him for saying nonsense thing that syring will let off syringe pain, but finally thanks him and ask for other syringe. He even dare to cut the visitor if needed.
Anyway he is a doctor and he almost solve the visitor's problem making the visitor happy at first and sad at last. Why should we live?
We should live to enjoy the life. Life and death are two sides of coin. These can't be detached from each other. So every person has to pass through these two sides struggling for the life part. When a coin is tossed, say a disease occured then either the flipping takes on life part or on death part. Sometimes the vertical position of coin make difficult to determine the side so it is termed Coma where life and death coexist.
We should live to enjoy the love. Love is part of life and without it living becomes tasteless like soda without salt. Life still remains without love but to live without love means death with in life.
We should live to enjoy the pains. Only the cowards choose death pathway as a immidiate solution to end the struggle. Only those can get extreme satisfaction who struggle for their existance and finally obtained the aim. Also we should live to enjoy happiness. Happiness an abstract noun termed opposite to life with pains which can and only can be felt after the pains. Happiness without pain is meager term for virtual comfort in life. Without pains the expression of happiness will be like a robot describing the composition of tear.
We should live to enjoy freedom. Freedom and life take you to heaven before the death. Bondage make life deteriorating both in quality and quantity such that it takes away a hope to survive. life becomes worse than hell. But there are those once in a blue moon who are able to find freedom in bondage ultimately enjoying the living and ending to heaven live Nelson Mandela.
We should live because we are destined to live. And one day when this destily leaves you you won't have chance to live. So lets live for our destiny.

Comfort
Patient feel comfortable when doctor is nearby and all his pains lessens. Is really pain reduces or just the feeling?
Pain is related with neurotransmitters. When the nociceptive stimulus arise the feeling of pain occurs and there will be increase in certain neurotransmitters. There will be further increment of such neurotransmitters if the patient become stressed, excited or anxious.
Comfort is the feeling which is without pain or with lessen pain than before. A patient feel comfortable in the presence of doctor may be related to decreased stress, decreased anxiety, increased tolerance to pain, hope of be cured etc. All these factor can stop the increment of pain transmitter thus making pain transmitter at constant level. The other factor developed in the due course like tolerance of pain, sensitization to stimulus within the body etc. reduces the feeling of pain although the neurotransmitter are transmitting at the same rate.

Do we believe god or depend doctor?
"God is a supernatural power" this has been taught to us from the time when we start learning the things. There is saying that God can do this and God can do that but nobody in real has seen in real life. Some says they have the feeling that god is somewhere around us. As an optimistic I too believe god.
"Doctor" is a profession name given to the person who are specialized in treating the disease. There is saying " god is a natural healer" . In contrast to that "Doctor" is patient's healer if he diagnose and treat it well, else may be manhandled as culprit to the death of the patient. So there is another saying "Doctor is not God" which implies God is superior than Doctor. But how can we find God when we don't know the address.
For that people developed temples, churches, mosque etc and wrote many verses of their saying as Gita, kuran, Bible etc . Yet people are not able to meet God and changed the term to feel god. This helped to all optimistic with a hope that someday they will meet god.
Doctors also depend upon God if he doesn't know the cause. For a simple example, what is the reason behing nasal polyp formation? many will argue about that thing. And as a conclusion says God knows!
Anyway what I mean is that we should believe God as well as depend on experienced doctors. Both of them are for our benifit. How to be a good doctor?
Doctor is a man of honour who dedicate his life to save life. But do every doctor doctor become ideal image for beginners in medical world?
Doctor is not simply a title, it is a glory due to respect and everyone with this should abide by certain ethics to be a role model. After spending lot of time,gaining enough precious knowledge along the ups and downs of each life corner, testifying the capability and efficiency, spending sleepless nights in search of the quest with dedication, sacrifice, frustration and desperation finally get nominated for that glory. Further practising and sharping the knowledge obtain the degree, "Doctor". So to be a doctor is hard job. And being a doctor maintain the honour is the hardest.
Unbiased caring is of utmost importance. Doctor should have sound knowledge of symptomatic treatment, etiologic treatment, palliative treatment, referrals etc. of illnesses and be able to categorised as it needs.Patient's life depends on this judgement.
When needed Doctor should cooperate with other collegues and disscuss the management and should carry out the treatment mixing own and expert's/collegues view for the best possible result. Patient's himself and his/her family cooperation and involvement in treatment eases to solve the problem earlier.
Knowing the patient illness, his/her feelings, opinions helps to decide which way to go in his/her management. Also post management cure and follow up should be emphasized as a prophylaxsis to prevent possible future damage.

Patient and expectation
Every patient who visits doctor wants quick recovery from what they are suffering. What might be the best approach for maximum benefit within less visit?
Patient suffering from acute disease needs a correct diagnosis to get the maximum benefit. Some doctors give only the symptomatic treatment which forwards the disease process to its chronicity. Also misdiagnosed cases may be fatal.
Patient suffering from the chronic disease needs a long term treatment and adherence to the treatment following the strict management rule. Its not that the treatment fails, but the carelessness of the patient or incomplete education of the patient about the treatment makes the management longer.
Patient expect that, after seeing the doctor for once, their discomfort go away and need not to bother the doctor again saving time and expense. But when they need the frequent visit, they feel they are being robbed.
Educating the patient plays a vital role in their treatment for the quick benefit. Doctor should be able to make them understand what they can do and what they can't. They should let them know the benifits and drawbacks of the procedure, and always use the one in which benefit is much greater than side effects. They should let them adherence to the treatment.
Many patient has habit of hiding the actual facts because they think doctor may unenvelope their privacy making them embarassed. So they expect total treatment with their superficial data. Doctor should know this fact and should skillfully make them speak out their actual data and maintain their privacy.

Doctor
What is your dream? A 4 year old boy replies "I want to be a doctor". Don't know who teached him to say like that. I wonder what would I have answered when the same question was asked for me at his age. But after I started to know things, as far as I remember, my answer was not different from him. The only difference was that I knew who is doctor.
My childhood was healthy except some mischevious cuts and stiches except once when I really coughed bad with runny nose, fever, malaise, muscle pain, bad appetite and weight loss. Involuntary cough seemed taking my lung out from the body and I felt worst. For the seek of comfort I visited hospital. After waiting for long time, I was taken to room where there were two other people, a man and a lady both with white apron, and asked me what was the problem. I just coughed and showed my runny nose, details was described by my mother. Then they took me to a bed which is walled by green curtains and let me sleep. The lady listened me using her ear device and told to have deep breath. After that she write some notes, and I heard they were saying that I was nearly to have Pneumonia, so need good care. That was the first time I personally know that the persons who are discussing about me were Doctors. And miraculously I don't have to eat full dose that I recovered.I was told to eat all the medications, and due to awful taste I hide some of them saying I had already eaten. Thanks for not havine the MDR species at those times.
After that I heard many news about doctors. Every family members talk about them. When I cry my father, mother and other family members comfort me saying future doctor don't cry; future doctor is brave person. In my family no one is doctor, but they always dream of one. They give many examples about goodness of doctor. May be the profession is good and is respected in my locality. Or may be doctors are rich and don't have to worry about unemployment. Or may be doctor is really a good profession. I wonder if they have inspired me for Pilot then I would be flying all over the world now; if engineer then building sky-high buildings. Infact I just know about doctor, engineer and Pilot in my childhood. Every teacher taught us to be one of them. I was never taught to be a teacher or writer. And whoever studied hard, they were praised as future doctor. It was a trend that every board topper go abroad for the further study in medicine. And every family was proud having son/daughter as a doctor.
Growing in that environment I just can think about Doctor, engineer and Pilot. Pilot has never been focused. Engineer is always out of the centre of discussion. The only option left gradually developed as my aim. And slowly it became my determination. Now I can proudly say that my aim is to be a good doctor. And it is in process.
As the above mentioned 4 year boy described and my aim is not different. The only different is that I now know what and how to be whereas former only influenced by surroundings.
