Osteoporosis in Older Men: Densitometry and Bisphosphonates Can Be Cost-Effective

Bone densitometry followed by bisphosphonate treatment for 5 years can be cost-effective in some older men, according to a computer simulation in the current JAMA.

Using data from epidemiologic studies, researchers examined the cost-effectiveness of screening for and then treatment of osteoporosis with bisphosphonates for 5 years. The model used 50,000-man cohorts with initial ages ranging from 65 to 85. Simulations were based on data from white men residing in the U.S.

The screening-and-treatment strategy achieved cost-effectiveness (cost per quality-adjusted life-year gained under $50,000) for men 65 or older with a history of clinical fracture. In the absence of a prior fracture, the strategy was only cost-effective at age 80 and older. If drug costs were halved, or if society were willing to pay $100,000 per QALY, the strategy would be cost-effective in all white men 70 or older.
