Hi friends,
As you know I have been in the Mechi Zonal Hospital, Bhadrapur, Jhapa...I have been looking the trends of cases enrolled here over the year. My group is extensively working on that. The final version will take few months to come out.
But I am really amazed to see the cases of Malaria (both P vivax and P falciparum) and poisoning cases especially Organophosphate one. The day before yestarday we saw three cases of rat poisoning: two small children of one family. There was one death of OP poisoning. So much so that doctors who are in turn of 24 hr duty wish not to have poisoning cases during their turn. But they invariably get them. I am really surprised to see these.
Next one is of Malaria, in the region where we expect Kala Azar, cases that too dominated by "falciparum" species. Yesterday, two of us were in the orthopaedics unit where there three cases of fracture and two of them were Bhutanese refugees, there there was call from Emergency to the on duty doctor who was with us. The we rushed there. There were two young boys (~22) flat on the bed. One of them was uncounscious and patient party were crying there and huge crowd developed!
In fact, these cases were brought earlier and a CMA had seen and sent blood for Malarial parasite. Both of them turned out to be positive for falciparum! Really whole lot of cases are found here. There is so huge global fund for TB, Malaria, Leprosy and others... but the cases in the grassroot .... as usual.
This is our ground reality.
