Fact About Kantipur Report: Bizarre baby born in Dolakha - 29-03-2006, 05:04 PM

This is a case of Anencephaly, I have encountered one in my life till now when I was posted in Gyn/Obs during my final MBBS. I have a series of photograph. Since anencephaly is not compatible with life, medical termination of pregnancy was done.

The “baby” weighed 2kg at birth and was born after the normal nine-month gestation period.

Suntali, already a mother of two normal daughters, was not suffering from any illness during the pregnancy.

Nir Bahadur, the father, says he does not feel any remorse for the newly-born baby’s death. “I am happy that nothing happened to my wife,” he said.
Fact about Anencephaly

What is Anencephaly?
Anencephaly is a defect in the closure of the neural tube during fetal development. The neural tube is a narrow channel that folds and closes between the 3rd and 4th weeks of pregnancy to form the brain and spinal cord of the embryo. Anencephaly occurs when the "cephalic" or head end of the neural tube fails to close, resulting in the absence of a major portion of the brain, skull, and scalp. Infants with this disorder are born without a forebrain (the front part of the brain) and a cerebrum (the thinking and coordinating part of the brain). The remaining brain tissue is often exposed--not covered by bone or skin. A baby born with anencephaly is usually blind, deaf, unconscious, and unable to feel pain. Although some individuals with anencephaly may be born with a rudimentary brain stem, the lack of a functioning cerebrum permanently rules out the possibility of ever gaining consciousness. Reflex actions such as breathing and responses to sound or touch may occur.

The cause of anencephaly is unknown. Although it is thought that a mother's diet and vitamin intake may play a role, scientists believe that many other factors are also involved.

Recent studies have shown that the addition of folic acid (vitamin B9) to the diet of women of childbearing age may significantly reduce the incidence of neural tube defects. Therefore it is recommended that all women of childbearing age consume 0.4 mg of folic acid daily.

What is the incidence of anencephaly?
1/1000 live birth i.e. one pregnant woman among thousands other pregnant will have baby with anencephaly in her womb.

How it can be diagnose?
It can be diagnosed by Amniocentesis, AFP, and/or an ultrasound between 14-16 weeks in couples with a history of an anencephalic infant.

What is done if a mother is diagnosed with anencephalic baby in her womb?
Medical termination of pregnancy because no way the child will survive after the birth as there is defect in the formation of brain and spinal cord.

Is there any treatment?
There is no cure or standard treatment for anencephaly. Treatment is supportive for the family.

What is the prognosis?
The prognosis for babies born with anencephaly is extremely poor. If the infant is not stillborn, then he or she will usually die within a few hours or days after birth.

What research is being done?
Many research includes studies to understand how the brain and nervous system normally develop. These studies contribute to a greater understanding of neural tube disorders, such as anencephaly, and open promising new avenues to treat and prevent neurological birth defects.

Oh my god!! I'm horrified...not because the baby has dysmorphic features but because of the way it's been handled in the society. Is it right to advertise the baby like that?? This couldn't more unethical...the poor parents are probably grieving over the loss of the baby and somone else is carrying it around parading the baby as it's somekind of alien for people to see. Some nepali people are so insensitive and need a kick up their arse!!

I also had exactly similar case in my gyanae posting in TUTH except the mother was primi and was from Lamjung.

well what about the role of a doctor?? do they not have a say in what happens to the baby? the poor mother. She's going to termed so many labels in the society such as being a witch for giving birth to a dysmorphic baby...shouldn't the doctors or the other medical people involved protect the mother and her dignity? It's none of the other people's business and the medical proffessional people involved should have tried to maintain confidentiality! - it's one the traits of being a doctor! this is a very issue for the medical people involved...what law does the government have for breech of confidentiality. In the UK i believe it would be warning from teh GMC if not suspension (Gardening leave!).

why educating the gerenal public. the doctors should have stopped the parading. it's serious misconduct of medical practice.

Well, actually I was also more shocked by the way it was carried than the case itself. And my first question to Dr Angel, as he asked me if I have already read the news day before yday, was "why is the baby carried this way?". I think the show is happening in front of the hospital. And the staff is showing the baby to curious ppl. But it is really bad that they took baby out to the public. They could have organized some kind of program within the hospital and shown it to ppl. But not outside in public. An I agree with what DD is saying. Imazine you give birth to an anencephalic, and your baby is shown to public in that manner. I would not tolerate that, it would kill my emotion.

The picture #2 above is beyond my imazination. Mainly the mass around the baby is shocking. If the baby was preserved in a glass jar in lab and ppl would have been watching it, that would be no problem. But the picture shows as if people have no feeling for the baby, and they are simply wondering the malformation, forgetting the mother-womb relation and respect for baby ( when a 10week old emryo has its right why not an anencephalic ?) I feel sorry for the baby and the parent, it was a bad luck.

i personally think the nepali medical council should disapline the doctors/ medical proffessional involved for such atrocious medical practice. This goes against the ethos of medicine.
