Case Study 1. Whiteout of the Left Lung - 03-07-2006, 05:57 PM

A 78-year-old woman with a prolonged history of chronic obstructive pulmonary disease is admitted to the hospital with an exacerbation of the disease and respiratory failure. What is the diagnosis?



Hint: The resident physician performed thoracentesis, which was unsuccessful.

There is a complete collapse of the left lung right sided lung is hyerinflated as the X-Ray showsh pitch black appearance.

It could be due to pneumothorax. Pneumothorax occurs when a hole develops in the lung, allowing air to escape into the space between the lung and the chest wall and collapsing the lung. Patients with COPD are at increased risk for spontaneously developing these holes because of weakened lung structure. A pneumothorax can lead to severe respiratory distress and is treated by inserting a tube into the space between the lung and the chest wall (pleural space) to allow the air to escape out of the space and re-expanding the lung. The tube must remain in the space until the hole is repaired.

All of you are correct dear friends! Its a case of left Lung collapse

Complete left lung collapse: The patient has a complete collapse of the left lung. On bronchoscopy, thick mucus plugs were removed from the left mainstem bronchus. In a complete lung collapse, complete opacification of the hemithorax is observed. The opposite lung hyperinflates, and the mediastinum herniates to the collapsed side. Misguided attempts at thoracentesis invariably fail, and urgent bronchoscopy is required. This radiograph also shows concomitant collapse of the right lower lobe.
