New cases in Pous 2064, HIV = 175, AIDS = 26, Death = 2. HIV rate is very high in Housewives than sex workers in Nepal ! ! ! HIV status in Nepal till 2005: Total Adult=70000, Adult Prevalence (15-49)=0.55%, Number of Women (15-49) LWHA=15,310 (22%), HIV Prevalence rate in IDUs=32.7%, HIV prevalence rate in sex worker=3.8%, HIV prevalence rate in client of SW=2.1%. 65 yrs old man..... - 13-03-2008, 04:43 AM

A 65 years old man was brought to the hospital who have had a low grade intermittent fever of unknown origin and unconscious weight loss for last three months. on physical examination hepatosplenomegaly was found. Auxiliary examination reveals normal chest x-ray, blood dyscrasia with pancytopenia. The patient shows negative tuberculin test. On biopsy of liver and bone marrow multiple granuloma like structure was found and depressed marrow function was elicited. what is the most likely diagnosis ???

since bone marrow function is depressed n hepatosplenomegaly is present there is some hematological problem............... Leukemia probably.........

Sort of a toss up. But from the two choices there, I'd lean toward Leukemia. It would fit all of the symptoms, and explain the negative tuberculin tests.

It's CML. Negative Tuberculin test so no TB. Multiple granuloma and pancytopenia mean leukemia. I don't think any other answer will fit into this question.

Most likely to be bone marrow tumor or with aplastic anemia.

A 48-year-old man who works as an accountant complains of weakness,
fatigue, and bleeding from the gums when he brushes his teeth. A complete
blood count shows severe pancytopenia with white blood cells (WBC)
1000/mm3, hemoglobin (Hgb) 8 g/dL, hematocrit (Hct) 24 percent, mean corpuscular
volume (MCV) 10 fL, and platelets 30,000/mm3. Bone marrow biopsy shows:

The answer is Aplastic anemia.
Aplastic anemia is defined as pancytopenia with marked hypocellularity of
the bone marrow.The bone marrow in this condition is actually hypercellular.
