What are the diagnostic criteria for Acute myocardial infarction?

We generally follow WHO criteria for the dx of AMI:
1) Typical chest pain or classical symptoms for more than 30 mins
2) ST elevation of at least 0.1 mV in 2 or more concurrent leads on ECG ( for Non STEMI: ST depression and/or T inversion)
3) Increase in cardiac enzyme levels

CLINICAL FEATURES OF MYOCARDIAL INFARCTION
Symptoms
Prolonged cardiac pain
Chest, throat, arms, epigastrium or back
Anxiety and fear of impending death
Nausea and vomiting
Breathlessness
Collapse/syncope




Physical signs
Signs of sympathetic activation
Pallor, sweating, tachycardia
Signs of vagal activation
Vomiting, bradycardia
Signs of impaired myocardial function
Hypotension, oliguria, cold peripheries
Narrow pulse pressure
Raised jugular venous pressure
Third heart sound
Quiet first heart sound
Diffuse apical impulse
Lung crepitations
Signs of tissue damage
Fever
Signs of complications, e.g. mitral regurgitation, pericarditis

At least 2 of 3 must present for the diagnosis of acute MI. (WHO criteria)

the Diagnostic Criteria for Acute MI are
1 [History] severe chest pain of >30 min associated with diaphoresis, N&V, collapse, syncope, cardiogenic shock
2 [ECG ] ST segment elevation or depression
3 [Biochemistry] raised CK-MB or cardiac specific Troponin
Either 1,2,3 or 2,3 or 1,3 criteria satisfies the Diagnosis of Acute Myocardial Infarction.
