HIV status in Nepal till 2005: Total Adult=70000, Adult Prevalence (15-49)=0.55%, Number of Women (15-49) LWHA=15,310 (22%), HIV Prevalence rate in IDUs=32.7%, HIV prevalence rate in sex worker=3.8%, HIV prevalence rate in client of SW=2.1%. The latest U.N. report shows that 65 million people have been infected with HIV since it was first identified 25 years ago. Twenty five million people have died of AIDS.

Pediatrics

Forensic Medicine
SECTION "A"
1. Define Medical Negligence (MN). Describe its basic elements. Explain the Doctrine of RES IPSA LOQUITOR and list five defenses which help to the doctor, charged for medical negligence in court of law.
2. What is consent? What is the difference between implied and expressed consent? Give the meaning and situation of informed expressed consent and Jehovah's Witness.
3. Write short notes on:
a. Delusion and criminal responsibility
b. Skeletal remains examination
c. Dying declaration
d. Blood and seminal stains as evidence in court.
e. Expert witness in court of law

4. What are the immediate and early changes in a dead body? Describe the mechanism of formation and medico-legal significances of hypostasis.
5. Define throttling. What are the mechanisms of death in manual strangulation? Describe the differences of post mortem findings in manual and ligature strangulation.

a. Contusion and medico legal significance
b. Finger prints as reliable tool for positive identification.
c. Entry and exit wounds from a rifled firearm weapon.
d. Coup and countercoup head injuries
e. Identification with the help of dental examination in living person and in a disintegrated body.

ENT

1. What is Waldeyer's ring? What is the significance of it? Write down the indications for tonsillectomy.

2. Enumerate different neck spaces. How will you manage a case of paraphyrangeal abscess?

3. Write down the etiology and clinical features of acute epiglottis. How will you manage a case of acute epiglottis?

4. What is tracheostomy. Write the indications for a tracheostomy and describe the complications one can get following the surgery.

5. Enumerate the different conditions for hoarseness. Outline the treatment plan for a vocal polyp.

6. Write short notes on: (any three)
a. Thyroglossal cyst
b. Adenoid
c. Intrinsic laryngeal muscles
d. Recurrent laryngeal palsy





OPHTHALMOLOGY

Section "A"
1. What is Accommodation? What does Presbyopia mean? How do you manage a case of Presbyopia?

2. List ocular manifestations of Herpes Zoster. How do you manage a case of dendritic ulcer?

3. A 60 year old female complains of sudden painful loss of vision in the right eye and present to Emergency Department with headache and vomiting. What is your likely diagnosis and why? How will you proceed with the management of the case?

Section "B"
4. Define cataract. Enumerate causes of Cataract. Outline Preoperative investigations and steps of cataract surgery.

5. What is Xerophthalmia? Mention WHO Classification of Xerophthalmia and its management in children and adults.

6. Write short notes on :
a. Ophthalmia Neonatarum
b. Pterygium



Community Medicine

1. A woman, 26 years of age, newly discovered as suffering from sputum positive pulmonary tuberculosis gave birth to a baby. Describe:
a. the category of DOTS treatment you would give to the woman
b. How will you protect the newborn from contracting the disease from the mother?
c. When would you declare the woman to be cured?
d. How will you find other cases in the neighborhood?

2. Define family planning? What are the reasons for its necessity describe the method of insertion of copper-T and its mode of action and complications.

3. How will you manage a class II dog bite? What will be your choice of vcaccin3e? Describe the dosage, method of administration, immunity development and complications of the vaccine.

4. Define occupational health? Describe the health hazard of doctors and nurses working in a multi-disciplinary hospital? How will you prevent the transmission of disease in the hospital?

5. Write short notes on:
a. National Health Policy 1991
b. Planning cycle
c. Staffing pattern of a health post
d. Causes of maternal deaths in Nepal
