possible mechanism for orthopnea,

cardiac compromise
increase venous return
increased pulmonary congestion
j receptors in pulmonary interstitium stimulated
stimulation of respiratory center
air hunger

yeh all do have certain role in orthopnoea,
but i think increased venous return is the main factor,
heart cant compensate for increased return which leads to pulmonary congestion.

clinical feature of PULMONARY EDEMA
1.cough
2.pink froathy sputum( pink colour is due to inappropriate mixing of leaked RBC from the congested vessel)
