For medical students those who come to Mainland China in English medium, they can take HSK Basic Examination. But in some medical school it's compulsory to take. And those students who are to study in Chinese medium, they have to take HSK Basic Examination before admission and have to submit their HSK Certificate for admission. Although HSK certificate has a permanent validity, whereas the accompanying test report will only be valid for two years (beginning from the date of the test session) as the certification for them to enter a college/university in China.
Basic Level Exams are held twice a year.

Personally I think HSK can't decide your skills on so called preinternship, but Chinese language is
VERY VERY important, because dealing with patient is all about by communication and you can't expect every patient (almost all maybe) to speak English and even though teacher might be there by your side while you practice but knowledge grows when you start working & gaining experience on your own.
