New cases in Pous 2064, HIV = 175, AIDS = 26, Death = 2. HIV rate is very high in Housewives than sex workers in Nepal ! ! ! HIV status in Nepal till 2005: Total Adult=70000, Adult Prevalence (15-49)=0.55%, Number of Women (15-49) LWHA=15,310 (22%), HIV Prevalence rate in IDUs=32.7%, HIV prevalence rate in sex worker=3.8%, HIV prevalence rate in client of SW=2.1%. The latest U.N. report shows that 65 million people have been infected with HIV since it was first identified 25 years ago. Twenty five million people have died of AIDS.

My Experience in Gynae OPD

In the 5th semester, we had spend all our time in the ward taking history but now we are in the 6th semester and recently we have been allowed to go to the OPD. I am posted in the gynae/obs dept. and as usual I was sitting in the OPD trying to learn how to deal with the patients as well as helping the interns by taking the BP, Wt., Ht., etc. then suddenly one young lady of 23 yrs old came to us and showed the urine report: pregnancy +ve.

We told her that she is pregnant but she didn’t believe us. She argued with us and told that she has done SURE Kit Test where –ve result was seen. I asked her “You are a married woman then why are you so tensed?” She told that she is already having a 7 month old child and so she doesn’t want another child soon.

We examined her and found 16 wks of fetus. We also saw the scar mark of Lower segment cesarean section (LSCS) and so we thought its better to consult our Mam, senior doctor and told her everything. Our Mam asked her about the reason behind not using the contraceptives for family planning and whether she was warned not to conceive for 2 yrs. or not as she conceived just 3 months after LSCS.

The lady outburst into tears and kept on crying continuously telling she doesn’t want this child. The intern requested her to do USG to find the exact gestational age as she was not sure about her LMP. She was so tensed that she told that she doesn’t believe any doctors and she boasted that she was not pregnant. She told that she will go to some other good clinic and abort this fetus.

I told her to be calm and not to cry. We are definitely going to give you good treatment and good suggestion. She looked at my face with the tears in her eyes and I felt so sorry for her but I could do nothing. While she was going, I told her everything will be fine, no need to worry. She smiled and told, “OK! Then I’ll do USG and again come back here with the report.” She went her way and once again we all became busy examining other patients…..

Even in the only metropolitan city of Nepal, every Gynae/Obs story that is told has potential to become a super hit tragic story.

You have yet to see, Khusboo. Keep your eyes open and ask your heart to go to sleep. This was just one little wave. Those who read this know what it is like out there but just ignore it. You know why and I know why, there is so little reason we should be discussing about this.
