Recurrent Pneumothorax .....
I feel bad when i get back to hospital and go to Bed No. 223, where there is a case of Rt. pneumothorax and Chest tube in situ. He had h/o of Rt pneumothorax and insertion of chest tube 1 yrs back and few days back he was admitted with c/o shortness of breath, On CXR-PA view we fould Lt pneumothorax and a minimal rt sided pneumothorax. We put chest tube in left side and after few days the Rt penumothorax increased in size and we had to put chest tube to rt side also.Patient had developed subcutaneous surgical emphysema and later needed subcutaneous incision also. Then we did pleurodesis Bilaterally. left pleurodesis was successful and rt failed. Later on the Rt pneumothorax was decreased, air was absorbed and we discharge him as Idiopathic Recurrent Pneumothorax. Now he again presented with Rt pneumothorax and we had again kept chest tube in right side. Pleurodesis was repeated 2 days back which again failed. We are unable to find the cause of repeated penumothorax so we labelled this as idiopathic. The patiend is not so tall and not so thin also.
So in total Rt pneumothorax 3 times, pleurodesis failed 2 times , lt pneumothorax 1 times , chest tube kept 3 times on right and 1 time in left.
Anyway, it is a challege currently to us now.
