Ejaculation
  • Muscle: Bulbospongiosus and Bulbocavernosus muscles with intermittent relaxation of external urethral sphincter and urogenital diaphragm.
  • Nerve: Sypathetic Nerves
Premature Ejaculation

What Is It?
Premature ejaculation occurs when a man reaches orgasm and ejaculates too quickly and without control during the sex. In other words, ejaculation occurs before a man wants it to happen. It may occur before or after beginning foreplay or intercourse. Some men experience a lot of personal distress because of this condition.

As many as one in five men experience difficulty with uncontrolled or early ejaculation at some point in life. When premature ejaculation happens so frequently that it interferes with the sexual pleasure of a man or his partner, it becomes a medical problem.

Several factors may contribute to premature ejaculation. Psychological problems such as stress, depression and other factors that affect mental and emotional health can aggravate this condition. However, there is growing evidence that biological factors can make some men more prone to experience premature ejaculation.

Rarely, premature ejaculation can be caused by a specific physical problem, such as inflammation of the prostate gland or a spinal cord problem.

Symptoms
The key symptoms of premature ejaculation include:
  • Ejaculation that routinely occurs with little sexual stimulation and with little control
  • Decreased sexual pleasure because of poor control over ejaculation
  • Feelings of guilt, embarrassment or frustration
Diagnosis
Premature ejaculation is diagnosed based on typical symptoms. To understand your problem, your doctor will need to discuss your sexual history with you. Be frank and open. The more your doctor knows, the better he or she can help you.

If your sexual history fails to reveal significant mental or emotional factors that may contribute to premature ejaculation, your doctor may want to examine you. Your doctor may examine your prostate or do neurological tests (tests of your nervous system) to determine if there is a physical problem that could be causing premature ejaculation.

Expected Duration
Sometimes, premature ejaculation goes away on its own over weeks or months. Working to relieve stress or other psychological issues may help the situation to improve.

Other men have lasting difficulties with premature ejaculation, and require professional help. Some men respond to treatment promptly, while others struggle with this problem over a prolonged period. Effective treatment is available.

Prevention
There is no known way to prevent premature ejaculation. However, you should consider the following advice:
  • Maintain a healthy attitude toward sex. If you experience feelings of anxiety, guilt or frustration about your sex life, consider seeking psychotherapy or sexual therapy.
  • Keep in mind that anyone can experience sexual problems. If you experience premature ejaculation, try not to blame yourself or feel inadequate. Try speaking openly with your partner to avoid miscommunication.
Treatment
Behavioral therapy is one possible approach for treating premature ejaculation. Most commonly, the "squeeze technique" is used. If a man senses that he is about to experience premature orgasm, he interrupts sexual relations. Then the man or his partner squeezes the shaft of his penis between a thumb and two fingers. The man or his partner applies light pressure just below the head of the penis for about 20 seconds, lets go, and then sexual relations can be resumed. The technique can be repeated as often as necessary. When this technique is successful, it enables the man to learn to delay ejaculation with the squeeze, and eventually, to gain control over ejaculation without the squeeze. Behavioral therapy helps 60% to 90% of men with premature ejaculation. However, it requires the cooperation of both partners. Also, premature ejaculation often returns, and additional behavioral therapy may be needed.

Another possible treatment is prescription medication that helps to delay ejaculation. Delayed orgasm is a common side effect of certain drugs, particularly those used to treat depression. This is true even for men who are not depressed. When this type of medication is given to men who experience premature ejaculation, it can help to postpone orgasm for up to several minutes. Drugs used for this type of treatment include selective serotonin reuptake inhibitors (SSRIs), such as fluoxetine (Prozac), paroxetine (Paxil) or sertraline (Zoloft); and tricyclic antidepressants, such as clomipramine (Anafranil).

There is also preliminary evidence men with premature ejaculation may benefit from drugs called phosphodiesterase inhibitors, such as sildenafil (Viagra), vardenafil (Levitra), and tadalafil (Cialis).

Some men with premature ejaculation also benefit from reducing the stimulation they experience during sex. A number of creams are available that can partially anesthetize (numb) the penis and reduce the stimulation that leads to orgasm. Another option is to use one or more condoms. However, these techniques may interfere with the pleasure experienced during sex.

When To Call A Professional
Speak with your doctor if you consistently ejaculate before you want to. Remember, one instance of premature ejaculation does not mean that you have a condition that requires treatment. Your doctor may refer you to a sex therapist if premature ejaculation is causing major problems in your sex life or personal relationships or if you would like to consider behavioral therapy.
Prognosis

Many men experience a brief period of premature ejaculation, then improve on their own. Even for men who require medical treatment, the outlook is usually good.

This definition in someway is not correct. I went throu diff literatures and found the proper definition of premature ejaculation--"ejaculation prior to the wishes of both sexual partners".
For example, if a man ejaculates in 7 minutes and his partner reaches climax in 5 minutes and gets satisfied in that 7 minutes, then this is not called premature ejaculation. Similarly, if a man ejaculates after 25 minutes but his partner needs 30 minutes to reach climax and to be satisfied, then this is premature ejaculation for her. Some women have delayed (more than hours) or difficult orgasm, and in this case nearly all men would be considered to have premature ejaculation.

What is premature ejaculation?

Premature ejaculation is the inability to delay ejaculation till the man wishes or when ejaculation occurs too early in the sexual act to satisfy both partners. The time to reach the stage of ejaculation is subjective and varies form one man to another. Some men may ejaculate immediately after copulation begins while another may not ejaculate even after 10 minutes. Thus, time is not a criterion for ascertaining whether the ejaculation is premature or not; mutual satisfaction is the benchmark against which this condition is measured.

What is the cause?

Premature ejaculation primarily has a physiological basis. Biologically, men have an orgasm approximately 2-3 minutes after penetrating the vagina. Women, on the other hand, typically take more time to reach the climax. Since in most cases, the partners may not climax together, they are not satisfied and the ejaculation is termed premature.

However, premature ejaculation may be caused by a variety of psychological and social reasons. It is seen that it occurs frequently when the man is under mental stress or anxiety. Anxiety may be due to the fear of non-performance, the fear of being discovered (as during premarital or extramarital sex in our society) or anxiety related to contraction of sexually transmitted diseases or an unwanted pregnancy.

Though men of all ages may experience the condition, it is more common in adolescents, young adults and inexperienced men. However, it is also true that almost all men experience it at some point in their lives. Premature ejaculation is one of the most common sexual problems for which couples seek advice. The result of treatment is often successful.

What are the symptoms?

Since premature ejaculation is not a disease, it does not have the classic "symptoms". An individual realizes that he is suffering from it when sexual intercourse ends before either partner is completely satisfied. It is diagnosed by a physical examination, in addition to interviews with the couple regarding their relationship.

What is the treatment? In some cases, the problem can be solved simply by educating an individual about the condition and reassuring him. Counselling is provided by psychologists that helps a man to deal with his fears and anxieties and, thus, eliminate the psychological causes of the condition. The following techniques may be beneficial in delaying ejaculation:
The "stop and start" method – in this method, the man learns to recognise the stage after which he cannot control ejaculation. The treatment method trains the person to remove the stimulus just before that stage is reached so that the urge to ejaculate is controlled. For example, when during masturbation, the man reaches a point just before ejaculating, he stops the stimulus until he starts losing the erection. Once the stage is past, he can resume the activity. This process is repeated again and again until the individual is able to delay ejaculation till the time he wishes. This method called the 'Masters and Johnson method', is most effective when the help of the partner is sought during actual intercourse.

The squeeze technique – in this method, the partner gently squeezes the tip or base of the penis just before the point of ejaculation thereby "cancelling" the orgasm. This process can be continued until the couple decides mutually to reach the climax.

Desensitising creams and gels are available in the market that reduce the sensitivity of the penis and help men reach climax later. Some men also feel that condoms reduce the sensations and, in addition to providing safer sex, help them last longer.

The couple can also experiment with sexual positions as some positions offer more control than others and may help to delay ejaculation.

HELLO SIR ,

what will you tell to a patient who comes to you with such problem???

will you directly tell that you ARE NOT GOING TO handle any such cases???????

IF THE SAME QUESTION WAS ASKED IN OTHER PUBLIC FORUM except MEDICAL FORUM .... THEN I WOULD HAVE SUPPORTED YOU 100%.

IT ALSO DEPENDS HOW THE QUESTION IS ASKED.

ANYWAY ......I JUST SAY YOU NEED TO BE LITTLE MORE BROAD/open MINDED AS A FUTURE DOCTOR.

Status Epilepticus is right at some point as he wants xenoMED to be out of vulgarity thanks to his concern, since the post is related to the education I also agree that we can discuss such matter openly.

Yes Oak, i didn't mean to say this definition was created by myself. To go into depth we need to follow different literatures. I just don't believe in "copy and paste" method.
